What this program does...
St. Joseph’s Healthcare Hamilton (SJHH) offers a broad range of outpatient health services and clinics that do not require an admission to the Hospital. Services within these programs include screening, assessing, treatment planning, group education, consultations, therapy and group support.
How this program helps...
A person-centered approach targets functional goals that are identified by patients and caregivers. Our objective is to align patient goals with treatment services focusing on best practices and achievement of desired outcomes. Individualized outpatient services are tailored to meet the specific needs of each patient.
