Haldimand Norfolk Assertive Community Treatment Team

What this program does...

Haldimand Norfolk ACTT delivers specialized community based services to people 18 years of age or older within the Haldimand Norfolk area.

We serve people with complex physical, psychological and social needs due to severe mental illness (including schizophrenia, schizo-affective disorder, major depression and bipolar disorder) accompanied by significant functional impairment, disruption of normal life tasks, periods of hospitalization and need for psychotropic medication.

How this program helps...

ACTT provides comprehensive individualized services to allow clients to gain or regain valuable and meaningful roles in the community.  Our services are based on the belief that all individuals have the right to full participation in community living.

ACTT uses evidence-based therapeutic principles and approaches with the key goals of:

  • Reduction of mental health symptoms
  • Increased housing stability
  • Heightened independence
  • Greater life satisfaction
  • Improvement in employment and educational outcomes
  • Enhanced social and family relationships

Your Care Team

The ACT Team is staffed by a multidisciplinary group of professionals who share the responsibility for treatment, rehabilitation, and support. The team includes specialists in psychiatry, nursing, social work, vocational counseling, addiction counseling, recreational therapy and peer support.
