Ensuring quality, patient safety, and exceptional experiences is our foremost commitment at St. Joseph’s Healthcare Hamilton (SJHH). Our dedicated team is extensively trained to continually enhance care delivery and operational management. We foster an environment built on equity, transparency, accountability, and trust, holding ourselves responsible to our patients and community.
By employing evidence-based decision-making, rigorous project management, scientific methodologies, ongoing performance monitoring and reporting (local, provincial, and national levels) and engagement with our patients and their care partners we identify areas for improvement that drive system-wide benefits and best practices that elevate our patient care standards.
To maximize our impact, we align our efforts with SJHH’s annual Strategic Plan, annual Quality Improvement Plans, Accreditation standards, and Choosing Wisely Canada guidelines. Our framework encompasses six key domains of quality:

Quality Improvement Plan (QIP)
The Ontario Government enacted the Excellent Care for All Act (2010) to prioritize patients by holding the healthcare sector accountable for delivering high-quality care. As part of this legislation, hospitals are mandated to create and publish annual Quality Improvement Plans (QIPs). These plans are instrumental in setting targets aimed at enhancing Safety, Effectiveness, Access, and Patient-Centred care across all hospitals in Ontario.
Hospitals are required to establish customized targets based on feedback from patient and employee surveys, as well as other data sources. This approach ensures that the plans are responsive to our feedback and are geared towards meaningful improvements. Continuous quality improvement is integral to our ongoing efforts to innovate and refine our practices, ultimately improving patient care, satisfaction, and clinical outcomes.

Measuring Our Performance
All programs and departments are required to consistently track, monitor, and publicly report on patient safety indicators as defined by the Ministry of Health and Long-Term Care. Our results can be accessed through the Ontario Hospital Patient Safety Measurements page. This data helps us pinpoint areas needing improvement, allocate resources accordingly, and implement sustainable enhancements for our community.
