Homes For Special Care (HSC)

What this program does...

HSC is part of the Ministry of Health and Long-Term Care province-wide residential care program for adults with serious mental illness. The HSC Program offers more than just residential group homes. Using a “partners-in-well-being” approach the program also assists people to explore and fulfill life expectations beyond psychiatric stabilization and health maintenance.

The Hamilton HSC Program oversees 9 independently owned and operated residential homes in urban and rural settings which support approximately 121 adults.

Also, we operate day programs in two Skills Centres located in Hamilton and Vineland where participants gain lifeskills, social skills and confidence managing daily living issues.

How this program helps...

HSC staff work with home staff to provide residents with a better quality of life and provide in-home assistance. For example:

  • Support activities of daily living (ADL’s) and medication management
  • Ensure residents have opportunities to participate in meaningful activities
  • Facilitate follow through and receipt of therapeutic services (e.g., counseling, vocational training, advocacy, referrals to obtain resources)

HSC staff conduct annual home inspections for licensing with the Ministry of Health and Long-Term Care and make recommendations for license renewals.

The Central South HSC Program also oversees two Skills Centres with designated staff. Skills Centre programs (e.g., prevocational, avocational, life skills, recreational/social) are developed according to requests and/or needs identified by participants. Activities may be on or off-site, in-home, individual or group-based. Programming is designed for participants to:

  • Acquire transferable skills within a safe, structured environment
  • Achieve their optimum level of functioning and well-being
  • Participate in mainstream activities and integrate into his/her own community
