The Stay Well Program – King Campus

What this program does

The Stay Well program is a physical maintenance program for patients with a broad range of medical conditions. This program offers therapeutic, safe and effective exercises for many at-risk patient populations including but not limited to: those who are post-stroke, have a history of heart disease, diabetes, renal failure, neurological disorders, such as Parkinson’s Disease, dementia and other various medical conditions.

How this program helps

The Stay Well program provides therapeutic exercises that focus on balance, strength, endurance and coordination in a medically supported environment. The primary goal of the program is to assist patients with maintaining and improving their physical function and continued independence. The Stay Well program is particularly helpful to those patients who have recently discharged from hospital and/or a rehabilitation setting and are encouraged to continue their physical function rehabilitation and recovery.

Your Care Team

The care team of the Stay Well program consists of Registered Kinesiologists, various Exercise Specialists and Assistants, a Medical Director and administrative support.

Referral Process

Anyone can be referred to the Stay Well program, however an accompanying medical clearance form must be completed and signed by an attending physician prior to acceptance into the program. Completed referrals can be faxed to the address provided above.

Preparing for Your Visit

Patients will be contacted for an initial assessment interview following receipt of referral. It is strongly recommended that a caregiver also accompany the patient to their initial assessment interview.

We ask that you do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.

Registration Procedures

Please go to the registration desk upon arrival. Please note, patients are required to pay for parking.

After Your Visit

Following the initial assessment interview, staff and patients will mutually agree upon the days and frequency of participation in the Stay Well program. There is no discharge from the program and patients are encouraged to continue in it given they meet the outlined admission criteria discussed in the initial assessment interview. The Stay Well program is offered in both morning and afternoon sessions Monday-Thursday.
