What this program does...

C-IMRAC (Community Internal Medicine Rapid Access Clinic) provides improved access to internal medicine consultation for patients who have highly complex and complicated medical issues. The General Internal Medicine (GIM) and interprofessional team at St. Joseph’s Healthcare Hamilton offers a unique opportunity to provide “urgent” access directly from the primary care providers to the consultants, eliminating the need for a visit to an emergency department. 

C-IMRAC extends the hospital into the community using the principles of patient-centered care for the management of highly complex chronic disease in a new and innovative model.

How this program helps...

C-IMRAC offers family physicians the opportunity of rapid telephone access to internists for advice and assistance at scheduled times daily. Physicians interested in this service, may call the clinic and will receive a booked appointment for telephone conversation.

C-IMRAC is designed to provide urgent on-site assessment for two groups of patients:

• Those with acute/urgent problems, who are unlikely to require hospitalization, but need assessment within 72 hours. Many of these patients may have undifferentiated problems that are difficult to diagnose or a poor response to current treatment.  Referral of these patients can be made directly with C-IMRAC.
• Patients with a history of repeat hospital admissions and frequent emergency department visits.  These patients have complex medical and/or mental health and addiction problems with frequent visits to the emergency department and hospitalization. These patients can be seen separately by an interprofessional health team with on-site internal medicine and psychiatry consultation, as needed.  The goal is to provide coordinated care that prevents the need for emergency room visits.
