Day Therapy - Chronic Disease Management Program

What this program does…

The Day Therapy Chronic Disease Management Program is a time limited program where you will learn how to self-manage your Diabetes condition. 

There are 3 parts to this program:

  • Individual assessments by each member of the multidisciplinary team.
  • Educational classes to help you with your Diabetes Management
  • Personalized exercise programs

The program runs Mondays and Wednesdays. Each day you may have individual appointments, attend educational sessions and participate in supervised exercise classes.

Our Day Therapy program encourages and supports the use of the most current knowledge and treatment practices supported by research and expert opinion.

How this program helps...

Through participation in the Day Therapy Chronic Management Disease program patients will learn:

  • How to set realistic goals
  • How to self- manage your diabetes with: Diet, lifestyle changes, medication
  • How to manage stress
  • How to exercise, eat well and maintain a healthy lifestyle

Your Care Team

Your care team consists of a diverse group of health care specialists as follows:

Social Worker (SW) will assist you by providing support around issues such as: stress finances, medication coverage, community resources and personal help.

Your Occupational Therapist (OT) will help to manage the difficulties you are having in performing the day to day activities that are important to you (such as washing, dressing, household activities). 

Your Physiotherapist (PT) will provide you with an individual exercise program to assist you in achieving your exercise goals.

Your Diabetes Nurse Educator will take your blood pressure and have you test your blood sugar at each visit. One–on–one appointments can be arranged to further review your diabetes management.

Your Recreation Therapist (TR) will help you find and /or continue leisure activities you enjoy.  The TR will talk to you about things you can do in your home and in the community to help you get involved and stay active.

Your Dietitian (RD) will help you learn how to manage your Diabetes by choosing healthier foods and will work with you to create a balanced meal plan.

Your Pharmacist will review your current medications with you and answer any questions you may have about how your medications work.

Referral Process

You will be referred as an inpatient, from the emergency department, from the Internal Medicine Rapid Assessment Clinic (IMRAC), ext. 37300 or other diabetes clinics.

Preparing for Your Visit

You must arrange your own transportation to the Day Therapy Program. You can drive or come by DARTS, bus or taxi. A family member or friend may also drop you off and pick you up.

What to bring with you:

  • Your Health Card
  • Your Medications or insulin (if required)
  • Blood Glucose Meter
  • Snacks and bottle of water
  • Please wear non-slip shoes and comfortable clothes that you can move freely in.
  • Please bring any equipment you may require to get around such as a cane or walker.

We ask that you do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.

From the Hospital Main Entrance: Go to the Dowling Wing Level 0 room D015-2 or enter at Martha Wing entrance on Charlton Avenue and proceed to Bishop Dowling Level 0 room D015-2. Please report to Day Therapy Reception.

After Your Visit

All recommendations for home care will be discussed during your day therapy visit. Please note that reports may be forwarded to other health professionals at your request with a signed release of information.
