Internal Medicine Rapid Assessment Clinic (IMRAC)
What this program does...
The Internal Medicine Rapid Assessment Clinic (IMRAC) is an outpatient clinic with a multidisciplinary team, located at the St. Joseph’s Healthcare Hamilton (SJHH) Charlton Campus.
IMRAC provides improved access to medicine consultation for patients who have complex medical issues. The General Internal Medicine (GIM) and multidisciplinary team at SJHH offers a unique opportunity to provide access for patients who have either been recently discharged by one of our Medicine physicians or to patients who have been seen in our emergency department and do not require hospital admission, but still require Medicine follow up.
How this program helps...
IMRAC provides timely access to a clinic for Medicine patients. Patients may be referred to IMRAC one of two ways:
- A patient has presented to the emergency department and the physician makes a referral to IMRAC. The patient will be assessed by the emergency physicians who will determine if it is appropriate to be discharged home if they can be seen in IMRAC in a timely manner (usually within a week, depending on scheduling).
- A patient has been discharged from one of the inpatient medical units and the physician believes the patient requires additional follow-up. Having IMRAC follow up may allow patients to be discharged earlier and may reduce the need for readmission.
Clinic appointments are scheduled either by clerks in the department you are discharged from or from the IMRAC clinic itself. Information regarding your upcoming appointment will be written on your discharge documents you receive from the hospital. This is called an After Visit Summary (AVS).
The physician and/or nurse practitioner will assess all patients who come to the clinic. They may arrange for patients to have some tests such as an x-ray, ultrasound, CT scan or blood tests. This is all arranged during the same clinic visit.
Patients may come to this clinic for 1 to 2 visits. The services are based on the patient's needs.
IMRAC is a multidisciplinary clinic and you may be seen by various healthcare team members as needed based on the teams assessment.
