What this Program does…

The Nurse Led Outreach Team (NLOT) is a team of registered nurses at St. Joseph’s Healthcare Hamilton who provide skilled, timely nursing services to Long-Term Care residents in the Greater Hamilton Area. 

How this Program helps…

Our goals are to:
-    Reduce visits to Emergency Departments 
-    Smooth transitions from Hamilton area hospitals to LTC
-    Facilitate access to urgent outpatient services 
-    Build nursing skill capacity in LTC

Our services:
-    In-home assessments and resident monitoring
-    Advanced nursing care, including: medication review, difficult foley insertion, PICC line maintenance and care, IV administration, clinical assessments, etc.
-    Facilitation of urgent diagnostic imaging (X-Ray, CT, ultrasound, etc.) and other outpatient services (e.g. specialist referrals)
-    Consultations in-person, by telephone, or email
-    Education and resources for registered and non-registered LTC staff
