What this program does...

Integrated Comprehensive Care (ICC) means the patient isn’t being handed off from one part of the system to the other, making points of transition in care seamless and less confusing for everyone.

Planning for home care after discharge from hospital starts before the patient arrives for their surgery, and throughout their care journey, patients have access to the same team. By connecting patients with the home care support they need before they leave the hospital, we’re setting the foundation for successful recovery. Once patients are home, they have 24/7 access to a clinician.

At St. Joseph’s Healthcare Hamilton, we currently offer the ICC program to patients who have received a hip or knee replacement, are undergoing surgery for lung cancer, Thoracic surgery, Cardiovascular surgery, Peritoneal dialysis or who have chronic disease such as Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF).

In 2019 St. Joseph’s Healthcare Hamilton will be expanding the ICC program to include all planned surgeries.

Patient Experience

One Care Coordinator
The Integrated Care Coordinator is a critical person in this model of care; this individual helps the patient navigate through every step of their journey, in the hospital and the community.

One Number to Call 24/7
Patients or family members can access the team on a 24/7 basis at any time during their care, by calling a central contact number. Our coordinators are on standby to answer any questions or concerns about your health.

One Electronic Patient Record
We maintain one electronic health record that tracks patients care from hospital to home. It eliminates patients repeatedly telling their health journey when meeting new team members.

One Health Care Team
Patients in the program are supported by a tight network of healthcare providers located in the hospital and the community. Members of the ICC team have access to a wealth of expertise, such as nurses, physicians, physiotherapists, occupational therapists, respiratory therapists, social workers, dietitians and speech and language pathologists. We can very safely transfer care from the hospital to more cost-effective care providers in the community because they are directly connected to a very knowledgeable team.

Virtual Care
The innovative aspect about Integrated Comprehensive Care (ICC) is that we’ve used straightforward technology to improve the delivery of care. By using tablets to maintain an electronic health record, patients and families can communicate with their health care team from the comfort of their home. Reaching another team member is only a phone call, Skype call or email away, and each team member is accountable to work with the patient and other members of the team. Not only does this give patients peace of mind and feel more confident but it also avoids unnecessary emergency department visits, and follow-up care can be delivered through virtual care..

By reducing the unnecessary barriers to receiving the right care in the right place, we are proving that this is a more cost-effective and efficient way of delivering care. We’ve been able to significantly reduce the amount of time spent in the hospital, which is the most expensive place to receive care, and deliver more care in the home, all while reducing the number of emergency visits after discharge from the hospital. Above all, our patients are delighted with the care they receive – they feel very well supported and are much less anxious about being discharged home from the hospital.
