Transitional Outpatient Program of Schizophrenia Service
What this program does...
TOPSS healthcare providers support people to transition from the Schizophrenia Inpatient Units (Waterfall, Orchard, Harbour North & East) to a community outpatient service.
We provide an active, evidence-based safety net for that vulnerable transition period from hospital to community.
How this program helps...
We are a transitional case management program.
A case manager connects with you during your inpatient stay, develops a therapeutic relationship with you and remains involved until you are established with a community care provider. The case manager has the flexibility to meet you in the community and hold telephone conversations during work day hours.
Your Care Team
You will continue to be followed by your inpatient psychiatrist and assigned a registered case manager.
Also, you will continue following the care plan established during your inpatient stay which may involve working with allied health providers (an occupational therapist and occupational therapy assistant, therapeutic recreationist, vocational counselor, psychologist and/or social worker).

Referral Process
TOPSS is a non-referral service accessed only by the Schizophrenia Inpatient Teams of the Schizophrenia & Community Integration Service (SCIS).
