Schizophrenia Outpatient Clinic (SOC)

What this program does...

The Schizophrenia Outpatient Clinic is an outpatient program for individuals living with schizophrenia or related psychotic disorders.

We provide a collaborative, person centered approach to assessment, treatment and rehabilitation. Our focus is self-management and recovery.

Your Care Team

Registered clients are provided a psychiatrist and another healthcare provider, referred to as the “Most Responsible Clinician” or “MRC”.  The “MRC” is the person you see the most and who you contact when you need support. The “MRC” could be a nurse, social worker, vocational counselor, recreational therapist or occupational therapist.

The MRC will:

  • Talk with you about your needs
  • Talk with you about your goals
  • Help you to get the right service, from the right people, at the right time
  • Support you through difficult times

If you choose, the "MRC" can coach you to set goals and work on self-management, reducing and managing the impact of illness on your daily life.

Referral Process

Referrals to this program are processed by Connect, St. Joe’s centralized intake service for Mental Health and Addiction outpatient programs. Click here to access the Connect referral form.

Preparing for Your Visit

Health Card

  • You must bring a valid health card to show at reception during each visit.

Interpreter Services

  • Please let us know if English is not the language you and your family members understand. We can arrange the services of a professional cultural interpreter free of charge.
