COVID-19 Update

St. Joseph’s Healthcare Hamilton is beginning to reintroduce surgical and outpatient services slowly using a gradual, phased approach.  Some individuals, based on the severity of their condition and care needs, are being scheduled for procedures and treatments.

If you are a patient, your care team will contact you with more information when you are rebooked or booked for your treatment.

Please come alone to your appointment to help us ensure physical distancing and to allow for patients to come to clinics safely. In special circumstances, the Manager may approve a support person to accompany a patient to Outpatient clinic appointments. Any exception to the visitor restrictions must be approved by the unit or clinic manager.

If you have concerns, please call your family physician. If there is an emergency, please call 911 and go the hospital immediately. If you have an urgent issue, please go to an urgent care centre. If this is a mental health emergency, call 9-1-1, the Crisis Outreach and Support Team (COAST) at 905.972.8338 or online at coasthamilton.ca; go to your nearest hospital Emergency Department. St. Joseph’s Healthcare Hamilton has Emergency Psychiatry Services at 50 Charlton Avenue East, Hamilton.

What this program does...

The Community Psychiatry Clinic (CPC) is a mental health outpatient program committed to providing evidence based care to those with serious mental illness over the age of 17.

All referrals receive an assessment to determine the treatment options that best meet the goals of the client and referral source.

CPC provides consultations, assessments, and offers a variety of evidence-based treatment modalities, including individual and group therapy.

CPC has a collaborative relationship with primary care physicians and other community based services.

CPC services are time limited. Upon completion you will be referred back to the care of your family physician.

How this program helps...

The Community Psychiatry Clinic provides:

Rapid Consultation Service

Primary care consultation to family physicians responding quickly to diagnostic, medication and treatment requests. Family physicians will receive a consult within 10 days of the initial consultation.

Borderline Personality Disorder Services

Clients diagnosed with Borderline Personality Disorder are assessed and offered DBT-based programming. This evidence-based treatment is offered in various intensities, with group therapy being the primary delivery method.

An initial assessment will determine a client’s treatment pathway within the Borderline Personality Disorder Services.

General Psychiatry

Clients with severe mental illness including bipolar disorders, depression, anxiety disorders, post-traumatic stress disorders and concurrent disorders who can benefit from time limited goal focused treatment which may include therapeutic groups (i.e. Cognitive Behavioural Therapy, Mindfulness, Behavioural Activation, etc), individual sessions, and psychopharmacology.

Your Care Team

Your care team may include psychiatrists, nurses, social workers, occupational therapists, psychologists, and peer support.

Referral Process

Referrals to this program are processed by Connect, St. Joe’s centralized intake service for Mental Health and Addiction outpatient programs. Click here to access the Connect referral form.

Preparing for Your Visit

Please bring any medication that you are currently taking. Having family members or friends accompany you may be helpful. We ask that you do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.

From the Outpatient Entrance, turn left down the first hallway. The Community Psychiatry Clinic is located on the left hand side.

From the Main Entrance, take the elevators or stairs to level 0. Exit the elevator and turn left. Turn left again down the first hallway. The Community Psychiatry Clinic is located on the left hand side.
