St. Joe's Healthcare Worker Vaccination Receipt

In order for us to move forward with the development of our mandatory vaccination program at St. Joe’s, we require all staff, physicians, learners and volunteers to declare their vaccination status with Occupational Health and Safety (OHS) by September 1, 2021. Our mandatory vaccination program will require all staff, physicians, learners, and volunteers to be fully vaccinated or be subject to the provincial directive of education and regular testing.

All staff, physicians, learners, and volunteers must complete an attestation form and submit their vaccination receipts by September 1, 2021.

It is no longer an option to defer or decline to declare your vaccination status. Those who deferred or opted not to declare their vaccination status will be contacted directly by OHS to update their attestation response.

The attestation form can also be accessed in one of the following ways:

  1. Digital screening tool (sjhhscrening.com): a notification will pop-up when you sign in
  2. Print copies:available at all HCW entrances, along with locked boxes where completed forms can be submitted.
  3. QR Code: available on posters across the hospital

You may download your vaccination record from the Ontario government website

3. You may also send a copy of the hard copy version of your vaccine receipt by fax to 905-521-6111 OR by inter-hospital mail to: OHS, Charlton Campus, Martha Wing, Level 4, Rm. H401.

All HCW medical documentation in OHS is maintained in strict confidence. The vaccination record will be used for outbreaks, and case and contact management.

Why is St. Joe’s instituting mandatory disclosure of vaccination status?

Ontario’s Chief Medical Officer of Health issued a directive mandating hospitals and home and community care service providers to have a COVID-19 vaccination program for employees, staff, contractors, students and volunteers. The Chief Medical Officer of Health was clear that this directive sets a minimum standard. The vaccination program must be effective no later than September 7, 2021, and at a minimum will require these individuals to provide proof of one of three things:

  • Full vaccination against COVID-19;
  • A medical reason for not being vaccinated against COVID-19; or
  • Completion of a COVID-19 vaccination educational session.

Individuals who are not fully vaccinated against COVID-19 will be required to undertake regular testing.

In order for us to move forward with the development of our mandatory vaccination program at St. Joe’s, we require all staff, physicians, learners and volunteers to do the following by September 1, 2021:

  • Declare their vaccination status with Occupational Health and Safety (OHS) by completing an attestation form
  • Submit their vaccination receipts
Why does Occupational Health and Safety want vaccine records from SJHH Healthcare Workers?

OHS is keeping records of healthcare worker (HCW) vaccine receipts for health and safety purposes to assist with outbreak protocol and to support case and contact management. These records will help keep our patients and healthcare workers safe.

If I’m currently working from home, do I still need to complete the attestation and submit my receipts?

Yes.

What will the COVID-19 vaccine records be used for?

The receipts will be maintained in strict confidence by OHS and be used if there is an outbreak to support case and contact management.

Is this confidential? Who will see this record?

All healthcare worker medical documentation shared with OHS is maintained in strict confidence. This information will only be reviewed by OHS, and will not be shared with your supervisor/manager or any other St. Joe’s employees beyond OHS. Managers will receive a biweekly report letting them know who has submitted a response, but the report will not contain what the response was.

Will my manager see the record?

This information will only be reviewed by Occupational Health and Safety’s Employee Health Team, and will not be shared with your supervisor/manager or any other St. Joe’s employees beyond OHS.

If I'm an employee at another hospital than St. Joe's, do I still need to submit my vaccine record to St. Joe's?

Yes. This information will only be reviewed by Occupational Health and Safety’s Employee Health Team, and will not be shared with anyone beyond OHS.

If the vaccination receipt for my second dose also lists my first dose, can I just submit my second dose receipt?

Please submit two separate vaccine receipts, even if your first dose is listed on your second dose vaccine receipt.

What if my initial attestation has changed?

If your initial attestation has changed (i.e. you selected “I am partially vaccinated” and have received a second dose), please complete the attestation again.

What happens if I do not submit my vaccination receipt by September 1, 2021?

If you do not submit your vaccination receipts by September 1, you will be considered unvaccinated and subject to the provincial directive of education and regular testing.

Will St. Joe’s make vaccination mandatory?

St. Joe’s vaccination program will require all staff, physicians, learners, and volunteers to be fully vaccinated or be subject to the provincial directive of education and regular testing.

We will consider mandatory vaccination if we are unsuccessful in achieving our goal in the spirit of staff and patient safety.

Full vaccination will be mandatory for all new staff, physicians, learners and volunteers without the option for testing.

Further details of the program will be shared on or before September 7, 2021.
