St. Joe's Healthcare Worker Vaccination Receipt

All St. Joe’s healthcare workers (HCW) who have received any COVID-19 vaccine (1st and/or 2nd dose) are requested to voluntarily send a copy of their Vaccination Receipt to the Employee Health, Occupational Health and Safety (OHS) Office. These records will assist with outbreak protocol, and for case and contact management. All healthcare worker medical documentation in OHS is maintained in strict confidence.

1. Have your vaccine receipt?

  • Take a picture of your vaccine receipt after 1st and 2nd dose

2. Misplaced your vaccine receipt? 

Complete the quick and easy form below. Once submitted, it will be sent to St. Joe's Occupational Health and Safety team for your employee record. 

You may download your vaccination record from the Ontario government website

3. You may also send a copy of the hard copy version of your vaccine receipt by fax to 905-521-6111 OR by inter-hospital mail to: OHS, Charlton Campus, Martha Wing, Level 4, Rm. H401.

All HCW medical documentation in OHS is maintained in strict confidence. The vaccination record will be used for outbreaks, and case and contact management.

We are voluntarily collecting vaccination receipts to assist with outbreak protocol, and for case and contact management.

All HCW medical documentation in OHS is maintained in strict confidence. Once your vaccination receipt is submitted, the details of your COVID Vaccine status will serve to maintain a record of vaccination for you as a St. Joe’s healthcare worker. This information will only be reviewed by the Employee Health team, and we are committed to protecting your privacy and respecting your confidentially. Your vaccine status will not be shared with your supervisor/manager or any other St. Joe’s employees beyond the Occupational Health and Safety team, and we will take reasonable steps to ensure that your information is safeguarded appropriately.
