Centre for Diabetes Care and Education

What this program does...

The St. Joseph’s Healthcare Hamilton Centre for Diabetes Care and Education provides assessment, education, treatment and support for people with diabetes, pre-diabetes, and people that are at risk of developing Type 2 diabetes. Our goal is to provide tools and techniques to manage diabetes and live a healthy lifestyle.

We are committed to ensuring that people with diabetes have all the resources and support they need to care for themselves with confidence.

Diabetes programs are offered at both our West 5th Campus and King Campus locations.

How this program helps...

You will learn about what diabetes is and how to manage it and care for yourself. The program is set up to help you meet all your educational needs. We offer one-to-one teaching and group sessions.

One-to-One Appointments:

  • Meet with the nurse educator and dietitian, who are certified diabetes educators.
  • Meetings are spread over several months to give you time to apply what you have learned.
  • During these meetings you will receive information about blood sugar testing, medications, activity, meal planning, controlling stress and preventing potential problems.
  • You will be able to talk about your concerns and ask questions.

Group Education Classes:

  • LIVING WELL WITH DIABETES

    For the individual newly diagnosed with diabetes, or as a refresher, this class provides a general overview and helpful hints on how to live a healthy life with diabetes.

  • CARBOHYDRATES

    Everything you wanted to know about carbohydrates and more. Learn which foods have carbohydrates and how counting carbohydrates can help control your blood sugar.

  • LABEL READING

    Confused reading food labels? This class will make you an expert in no time!

  • HEART HEALTH

    Getting to the Heart of Diabetes. Learn how to protect your heart. What you need to know about fats and fibre and which foods should be in your grocery cart.

  • DINING OUT

    Is it possible to eat healthy while eating out? This interactive class will teach you tips and guidelines to help you make healthy choices at a restaurant, food court or a friend’s house.

  • DIABETES AND CARING FOR YOUR FEET – Why it is so Important

    Learn how to care for your feet. Chiropodist present for discussion.

  • CRAVING CHANGE

    A how-to workshop for changing your relationship with food. Understand WHY you eat the way you do. Comfort yourself without food. Change your thinking, change your eating. Cost of workshop is $10 (includes price of workbook).

  • EATING WELL WITH DIABETES (West 5th Campus only)

    “In the Eating Well with Diabetes group session, a Registered Dietitian will cover topics such as healthy food choices, meal planning, reading food labels and how food relates to diabetes management.”

Referral Process

For one-to-one appointments with members of our diabetes team, your family doctor can send a referral, or you can refer yourself. Our diabetes care team works closely with you and your family doctor.

Preparing for Your Visit

Please bring to all of your diabetes appointments:

  • Current medications or list of current medications
  • Blood sugar meter
  • Log book of blood sugar readings
  • Your Health card

Please give 24 hours notice to cancel or change an appointment.

We ask that you do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.

Registration Procedures

Please bring your health card. You will register at the booking desk.

After Your Visit

After you have learned about diabetes and your blood sugar is controlled, you will return back to your health care provider for care.  You can come back to our program if your condition changes.
