What this program does...

Hamilton ACTT2 delivers specialized community based services to people 18 years of age and older within the Greater Hamilton area.

We serve people with complex physical, psychological, and social needs due to severe mental illness (Priority given to persons living with a diagnosis of schizophrenia, schizoaffective or other psychotic disorder) accompanied by significant functional impairment, disruption of normal life tasks, periods of hospitalization, and a need for psychotropic medications.

Hamilton ACTT2 provides comprehensive, individualized services to allow clients to gain or regain valuable and meaningful roles in the community.

ACTT uses evidence-based therapeutic principles and approaches with the key goals of:

  • Greater life satisfaction
  • Improvement in employment and educational outcomes
  • Enhanced social and family relationships

Your Care Team

The Hamilton ACTT2 Team is staffed by a multidisciplinary team of professionals who share the responsibility of rehabilitation, treatment, and support. The team includes specialists in psychiatry, nursing, social work, vocational counseling, addictions counseling, occupational therapy, and peer support.

Hamilton ACTT2 carries out its services in partnership with other area service providers, as well as physicians, family members, and existing support networks

Referral Process

Anyone, including family members and potential clients, can refer an individual to our services by calling our office. Admission to an ACT program is based on an assessment of need and service capacity.

Preparing for Your Visit

An appointment will be arranged for you to meet with representatives from the team including the psychiatrist.

Any information that you may feel is helpful for ACTT to know should be brought with you along with a list of medications.

Having a support person or family member with you is also helpful.

We ask that you do not bring any valuables with you as the hospital cannot be responsible for lost or stolen personal items.
